audit medical records for evidence of information
supportive of IRF-PAI data including, IGC assignment, comorbid
conditions and functional modifiers and scores;

review forms such as FIM flow sheets and
their effectiveness in gathering relevant data;

audit staff utilization of facility forms;
-
audit effectiveness in capturing tier
assigning comorbid conditions;
-
evaluate facility tier distribution in comparison
to national benchmarks;
-
provide a listing of the most common tier assigning comorbid
conditions, by tier, by RIC;
-
recommend changes to forms, if appropriate;
-
present findings of chart audits to the working
group - including strengths and opportunities for improvement;
-
provide remedial training focused on selected
areas of weakness or non-compliance, if needed;
-
design brief quizzes to assess staff reliability;
-
address staff scoring issues or questions,
provide study sheets for FIM items;
-
conduct training workshop - half-day or full day
consult with on-site trainers.
