IT HealthTrack was established to promote quality in healthcare by providing facility administrators with valid and reliable data collection and management services for measurement and improvement of follow-up outcomes related to patient functional performance.

  • Promote the use of uniform definitions and measures of limitations of individuals’ daily activities and participation in social roles:

  • Protect and secure individually identifiable patient information from unauthorized disclosure;

  • Assess durability of outcomes over time and in each person’s daily environment;

  • Administer valid instruments in a reliable manner by healthcare professionals with clinical experience;

  • Utilize the latest information technology and telecommunications for innovative services and products;

  • Manage healthcare data with integrity and scientific rigor;

  • Promote in our staff a work ethic that rewards excellence, responsibility, and loyalty to our customers and to each other;

  • Conduct research activities that contribute to the body of knowledge in the area of interdisciplinary physical rehabilitation;

  • Disseminate information to our customers that evaluates their program performance, provides a comparison to benchmarks and results in targeted actions aimed at improving quality of care for individuals served. 

  • Assess post-discharge patient functional performance using standardized instruments with widely accepted measures and established psychometric properties;

  • Collect and aggregate uniform data to establish follow-up benchmarks for comparisons of outcomes;

  • Use vigilance in the development of methods for data transfer protecting and safeguarding patient and facility healthcare information;

  • Provide telephonic data collection by a professional staff trained in reliable administration of instruments and sensitive to the privacy of individuals;

  • Produce reports that are timely, relevant and useful for facilities to monitor their outcomes and identify opportunities for improvement;

  • Continually strive for excellence in customers service with regard to accessibility, knowledge and responsiveness;

  • Facilitate our customers’ goals to provide evidence-based practice, meet standards for  accreditation in the area of follow-up and achieve success with their marketing plans;

  • Provide services with demonstrated value when comparing products and services to costs; 

  • Develop strategic partnerships with our customers in their quality improvement programs.
