IT HealthTrack has a contractual agreement to
function as a business partner with the hospital
-
IT HealthTrack receives from the hospital only
minimal information – just enough information to fulfill our
contractual agreement
-
IT HealthTrack is bound by contractual agreement
to safeguard and protect patient information against unauthorized
disclosure
-
IT HealthTrack is a national leader in telephone
data collection of post-discharge functional status for rehabilitation
hospitals
-
Our nurses performing patient interviews have
many years of experience in clinical nursing practice
-
The questions our nurses are asking are based on
a standardized instrument called the Inpatient Rehabilitation Facility
– Patient Assessment Instrument or IRF-PAI
-
Data derived from your interview is returned to
the facility so that hospital administrators can determine, on
average, how independently their patients are functioning three months
after being discharged from medical rehabilitation
-
Nurses may also have asked you or your family
member questions about your satisfaction with certain aspects of the
rehabilitation program or facility – this provides you with another
opportunity to voice your opinion
-
IT HealthTrack provides rehabilitation hospitals
and/or units with reports comparing an aggregate of their results or
outcomes with that of similar facilities.
